The SCDF COVID-19 Emergency Fund will focus on the need in our Sickle Cell Disease Community in California. This fund is to aid with costs related to food insecurities and small household items (cleaning supplies, toiletries, etc.) during the pandemic.
If you are an individual with SCD or a caregiver of a person with SCD who has been directly affected by Covid-19, you may be eligible for assistance.
To be eligible for assistance, client MUST BE a resident of California.
Proof of diagnosis will be required for any applicant who is not a client of SCDF.
This fund will not cover mortgage payments, rent, bills, etc. (Assistance is available with finding additional resources not covered by SCDF).
An Emergency Request Application must be filled out by client or caregiver.
If you are not a current SCDF client, additional forms will be needed.
Once application is reviewed, a SCDF Community Health Worker will contact you for additional information.
Submission of this application does not guarantee an approval.
If approved, please allow 14 days to process your request.
If approved, a VISA Gift Card will be sent via mail to the home address on application.
Applicants can submit amaximum of 2 applications for this program. The second application can ONLY be submitted 60 days after first application submission.
Due to high demand and funds availability, repeat applicants may be placed on a waiting list, at which time you will be informed by our Community Health Worker.
The continuation of this program depends on the availability of funds.
