LifeSteps Transition Program

For Youth & Young Adults with Sickle Cell Disease

The Sickle Cell Disease Foundation of California has developed an intervention program that helps young people with sickle cell disease transition into adult healthcare in Los Angeles County.

Individuals with sickle cell disease who are ages 15-18 and their parents are encouraged to participate.  Individuals who are over 18 and have not yet transitioned to an adult provider are also welcome to register for this program.

The LifeSteps Transition Program consists of four workshops and a family retreat.  The timeframe runs from June through March.
