Community Health Workers
Health Navigation:
Provide SCDF members with information, training, referrals, & support.
Assists SCDF members in:

Accessing appropriate primary and specialized health care.

Understanding the health care system or engaging in their own care.

Connecting to community resources necessary to promote a member’s health.

Addressing health care barriers, including connecting to medical translation/interpretation, transportation services, or addressing health-related social needs.
Health Education:
Promote SCD Health and/or address barriers to physical and mental health care, including providing information or instruction on health topics.
Health education may include coaching and goal setting to improve SCDF Members’ health & support with the ability to self-manage their SCD.
